The IWS is only accessed through self-referral. You can self-refer by clicking the link below. Parents/carers and/or professionals can support you to make this referral, however would ask that they do not complete it for you.

Before filling out our referral form, please read Noah's Ark Centre's Data Privacy Notice (click here) and make sure you have to hand your address including postcode, contact phone number and email address.

When asked which service you are referring yourself to, please select "Integrated Wellbeing Service (15-25) Park & Ovenden" for all options (e.g. NOT the individual option you are wanting to choose - we will discuss this when we contact you).

Once you have submitted your referral, the IWS Coordinator will be in contact with you within 2-3 weeks to assess your needs.

We aim to assess and allocate new referrals within a short time frame. There is currently up to a 4-6 week wait, from the point of assessment to you beginning counselling, depending on your availability to attend sessions. If demand increases this wait may be longer and we will update time scales here accordingly.
